We appreciate your participation in the ASU-WFU Summer Institute Team Based Patient Care course! As you may remember, the course developed as part of a HRSA grant. Now as the grant is coming to a close, Wake needs to report comprehensive data on all participants.

At this time, neither the Health Professions Advising Office nor the Wake Forest Summer Institute staff have current information about your pursuit of becoming a PA.
We ask that you complete this Summer Institute Follow-Up Form by Wednesday, September 15th for the grant report.
